This is what the final result will look like.

Create a new document in Photoshop as shown below, 500 pixels x 500 pixels.

Create some guides for our top ribbon layer

Select the Rectangle tool with a grey color and draw a rectangle as shown below.

Press ctrl-t (PC) or cmd-t (Mac), then hold down shift and rotate and resize the rectangle into place as shown below.


Select the Triangle layer so that it is selected, then copy it by pressing ctrl-t (PC) or cmd-t (Mac)

Select the Path selection tool

Then select subtract from shape are icon as shown below

You can now deselect the Triangle layer as you won't need it anymore, and you should end up with the following

Select the rectangle tool and a slightly darker color, then draw a rectangle as shown below.

Move it down and left a bit.

Select the rectangle tool and a draw a shape as shown below, using a slightly darker color for distinction.

Zoom in, then turn your guides on, and select ctrl-t (PC) or cmd-t (MAC).

Zoom in even further, and whilst still in Skew mode, drag the lower right corner up as shown below.

Make the top and bottom layer the same color, make sure the bottom layer is at the bottom, and the Fold layer is in the middle and you should end up with the following.


Download a paper texture layer.
I downloaded a paper texture layer from here.
Then, open the texture in a new document in Photoshop, select all by pressing ctrl-a (PC) or cmd-a (Mac), and the go to the Edit men and select define pattern.

Then, right-click on the Top Layer, select Blending Options and choose Pattern Overlay.

Select overlay pattern for your bottom layer too and you should end up with something like this.

Right-Click on the Top Layer, select Blending options, and apply the following settings.

You should end up with this.

Create a new background layer, right-click and select Blending Options.
Then, select Color Overlay and choose #3b3b3b as the color.

Then, download some wooden textures from here.
Then, open a texture you like, paste it on a new layer above the Background layer, and give it a mode of overlay with a percentage you like.
Finally, create some text on your new layer. You can create a see-through effect by selecting a portion of your background and defining it as a pattern.
Then, apply the following settings to your text layer, by right-clicking on the text layer and selecting Blending Options.



Finally, knock down the opacity on your Top and Bottom teture overlay layers, and tweak the colors.
